Collaborative Heart Failure Management: Palliative Care and the Patient with Advanced Heart Disease

Abstract: A collaborative program between cardiology and the palliative care team at Mount Sinai Hospital was created to care for patients with advanced and end-stage congestive heart failure. We followed 30 patients for feasibility of the program. Results showed it is a feasible program and provided practitioners with new tools and understanding of managing this complex patient population.
