Join experts in the implementation of integrated care for a guided full-day symposium hosted by the Institute of Health Policy, Management and Evaluation. Participants will leave with next steps to implement integrated care in their own health teams.
Focused panel discussions will include:

  • population-based integrated care
  • patient and caregiver centered care
  • data driven change
  • provider engagement
  • governance, implementation and evaluation

Parking  Parking is available at the BMO Education & Conference Centre via two lots off of Nassau Street. 
Registration Fee $350 plus HST. The fee includes refreshments throughout the day, a catered lunch, and wine and cheese reception.
In Event of Cancellation In the event of cancellation before the symposium begins, the registration fee will be refunded less a $150.00 administration fee. Cancellations must be submitted in writing. Substitutions are allowed.
