The Future of Integrated Care with Guest Speaker Ed Wagner
IHPME is pleased to host award winning epidemiologist and former Director of the MacColl Centre for Health Care Innovation at the Kaiser Permanente Washington Health Research Institute, Dr. Edward Wagner, for a lecture and discussion on the future of integrated care in our health systems.
Dr. Wagner has extensive experience in care coordination initiatives, having developed the Chronic Care Model (CCM) with colleagues at the MacColl Centre, as part of the Patient-Centred Medical Home (PCMH) and Care Coordination Model, which sought to improve primary care nationally and internationally.
As a retired physician and epidemiologist, Dr. Wagner has continued to work extensively in devoting his time and efforts to improving services for young people in crisis and adults with mental illness.
This is an opportunity for all faculty and students to engage with a seminal leader in integrated care policy.
