Jatinder is the Director of Community Programs and Integrated Care at Southlake Health, in Newmarket ON. Jatinder’s provides strategic leadership and operational oversight for Southlake’s community and integrated care portfolios, including Southlake@home, Southlake@home Plus, Blue Door Inreach, AHF units, Nurse Led Outreach, Hospice Palliative Care, and the Extensivist Clinic, while advancing Southlake’s Strategy for the Decade Ahead and our evolution toward a Distributed Health Network.
Jatinder is an accomplished healthcare leader and regulated health professional with more than 20 years of progressive leadership experience across community-based care, rehabilitation, ambulatory services, and complex acute care systems. He is recognized for strengthening patient flow and transitions, improving access, and building integrated service models that enhance quality and patient experience. Prior to his current role, Jatinder served at London Health Sciences Centre as Vice President, Education, and previously as Vice President, Clinical Programs (Children’s Hospital & Women’s Health), providing strategic and operational leadership across large, multi-site portfolios. He also brings deep operational experience from leadership roles at University Health Network, West Park Healthcare Centre, Bridgepoint Health, and Rouge Valley Health System, where he led service redesign, performance improvement, and interdisciplinary program delivery.
