Performance Measurement, Quality Improvement and Continuous Learning
Teams are accountable for patient outcomes and continually finding ways to improve care delivery.
What it means for OHTs?
The care provided by teams will be based on evidence-informed practice and clinical standards. Emphasis is placed on continuous quality improvement to ensure best possible outcomes. In order to appropriately plan and utilize resources for the patient population teams must have the ability to analyze data.
