Documents
- Transcripts
- Transcripts must be scanned and uploaded to the SGS application website from all post-secondary institutions. Official transcripts will be required if you are selected to move forward to the interview stage.
- Grade legends and translations should accompany international transcripts.
- If you do not have a transcript from your residency program you can provide a letter from your residency program outlining the details of your training (how long and where). We will use this letter as a transcript for your residency.
- The letter of intent should be concise and relevant. Think about addressing your previous education, reasons for wanting to enrol in CEHCR, future career goals and research interests as they apply to CEHCR. It is also a great place to elaborate on any funding or supervision arrangements and clinical obligations during your studies.
- Curriculum Vitae (CV)
- Proof of funding and protected time from the clinical program, fellowship director or employer (these are required after acceptance into the program).
CEHCR applicants will need to complete a background information/additional information form which can be downloaded from the SGS application portal (after submitting your application). This document must be completed and uploaded by the deadline for your application to be considered.
Supervisor
It is not necessary for applicants to identify a potential supervisor on their own. However, it is often helpful for applicants to discuss potential projects with an expert in their area of research, as this will help propose a worthwhile study in the application. If applicants are able to make contact with a potential supervisor, this would strengthen the application. Review Faculty Profiles (as well as their areas of methodologic expertise) and Research and Initiatives to find potential supervisors that align with your research interests. Please make sure that you have met all of the application/eligibility requirements before contacting potential supervisors.
Reference Letters
Two confidential letters of reference are required from academic and/or professional supervisors or other appropriate contacts. The application process includes supplying institutional email addresses of the two referees that will receive an email with a link to an online reference letter form.
